I noticed this as well. Looks like it’s meant to cover the entire screen, for the poison damage. At least the actual damage works.
It sometimes appears in fullscreen, but still not entire screen in ultra-wide, just 16:9.
Same for the fire-effect, btw. (flames on the bottom of your screen).
