Game randomly changes equipped ammo to an accessory

Steps To Reproduce:
It’s random when it occurs, but happens during a reload of an empty magazine.
Has happened a couple of times with Kpist using an Acog, Extended mag., 9mm FMJ, Suppressor.
Shot out entire mag, and it converted my remaining bullets to an extra extended mag.
I can then take of and insert both Kpist Extended Mag. When I restarted the game, the extra Extended mag was gone and all of the ammo. I lost all ammo equipped on weapon, and all of the same type of ammo in inventory.

It happened again, same way. But this time it duplicated the Acog sight. I shot until the magazine was empty, and then the remaining attached ammo disappered.

A similar situation on ps4.Only happens to the small and assault wepons when i put a scope to it.
