I can remember that happening to me same place and same thing picking up an item.Its not happend to me anywere else since thankfully.

did this happen straight after updating ?
pls try the following.
start game and from menu select the character (not using continue option) load and then exit , then restart and see if the problem still exists.

No there was no update at the time as iā€™m pretty new to the game happened 3 days ago
