“Should contain” doesn’t mean 100% every single time there is something in it. If devs are sure that there is item every single time in loot containers, that line would be:"… gameplay will contain at least one item …".

I’m sure every loot container is supposed to contain loot. Like every building is supposed to be represented on the map. The off-cases are very likely mere oversights, unless there is something story or context related to it.

“Should contain” is a statement here of intended function but without the promise that it will behave perfectly. It’s typical in tech docs to hedge a little since they evidently can’t guarantee perfection.
