Steps To Reproduce: Happens frequently during combat, but I’ve been unable to come up with a consistent method of reproduction.

Players in your game: 1
Pressing Triangle to change weapons, or the D-Pad to change to a quick slot item, will show the white box surrounding the selected weapon or item, as per usual, but you will not actually switch to it.
Issue still exists post v1.03
