My first guess would be “quick resume” if you’re playing on an a Xbox Series X|S console.
If you didn’t exit the game via the main menu but just shut the console down / put it in stand by then it might not trigger an autosave.
Relaunching the game from quick resume kicks you out and forces you to log back in and if it hadn’t saved then you’d reload the previous save.
